Deposit fee of Rs. 2,000/- (non-refundable) for positions Sr. #01 & 02, and Rs. 1,000/- (non-refundable) for positions Sr. #03 & 04 as per challan instructions. For Sr. #05, send application along with passport size picture, updated CV, copy of valid CNIC and Demand Draft/Pay Order of Rs. 500/- (non-refundable) in favor of Punjab Healthcare Commission at Head Office Address. All online applications must accompany a scanned copy of the paid challan form. Internal employees/government/semi-government/autonomous bodies employees should apply through proper channel with fresh NOC.
